In America, this game is called Tetris Attack.  This game has several 
ending songs, but this is the one you get in "Stage Clear Mode".  

This is a transcription of the guitar part, followed by the melody,
separately.  (I'm not sure what instrument the melody is.  Some wind
thingy?)

I don't know if anyone cares, but I just went and made a GP5 for this, which I put as the download link. I was thinking that'd be a first step to making a solo arrangement ^-^;
