On the second to last line, I think the second half should be the same as the first but an octave up.

Those are my comments on the notes. As far as how you play them, you could move more things onto other strings so you don't have to move around your hand as much. The only part that really needs it is when you go from fret 3 to 15 on one string in one line, but it's slow enough it's playable anyway ;-) Up to you.
