That's called your avatar. Just go to your User Control Panel -> Profile -> Edit Avatar and upload it.

That's just what it says there by default. I think I can make it so that you can put whatever you want there though.

Ok, I have figured out how to get custom user titles. Just go to your control panel, profile, then fill out the little field at the bottom that says User Title. Enjoy.
